1. What procedures are discounted?
All dental procedures are discounted. Samples of the discounted
fees appear on the sample savings page.
2. What do I show to get my discount?
You must show a valid picture ID along with your Dental Club
card.
3. What if the procedure my dentist recommends is not shown on
the partial list of fees?
Ask your dentist what the FDH Dental Club fee is for the
recommended procedure. If the procedure is not a fixed amount by
contract, your dentist will discount their fee by 15%.
4. What if I need to change my individual membership to family
coverage?
Fill out the change form included in your member enrollment
packet and remit with the payment specified for the family upgrade.
A new ID card, listing your new dependent(s), will be mailed to you.
5. How can I get my dentist to join FDH?
You can nominate your dentist online here, Nominate your dentist,
or contact our Provider Relations Department at 888-334-5463.
6. When is my membership effective?
Upon receiving your card, you are eligible to go to any
participating Dental Club and start saving!
7. If I have a family membership, does everyone have to go to the
same dentist?
No, each member may see a different Dental Club dentist and/or
change dentists as often as they wish.
8. What constitutes a "Family Membership"?
Family members are living with or are financially dependent upon
you.
9. What is the cost?
$ annually for an individual membership. $ annually for a family
membership.
